
Steps to Create a Survey in Intellia :
1. Go to the โSitesโ Section:
In the left-hand menu, click on Sites.
From the drop-down, select Surveys
2. Create a New Survey:
Click on + Add Survey in the top-right corner.
3. Survey Builder Interface:
You will be taken to the survey builder interface. Similar to the form builder, you can drag and drop different elements to build your survey.
The available survey elements include:
First Name
Last Name
Phone Number
Radio Buttons
Checkboxes
Dropdowns
Text Field
Date
Paragraph (for longer text answers)
4. Add and Customize Questions:
Drag and drop the question types you want to include in your survey into the survey builder.
Click on each field to customize the question label and answer options (if applicable). For example, radio buttons or checkboxes can have predefined options for users to select.
You can add as many fields or steps as you need for your survey.
5. Add Multiple Steps (Optional):
If you want your survey to be multi-step, click the Add New Step button at the bottom of the current survey step.
You can create a sequence of questions, and users will move through the steps as they answer.
6. Customize Survey Settings :
Click on the Options tab at the top right to access the survey settings.
Survey Name: Give your survey a name (for internal use).
Redirect on Submit: Set a thank-you page URL or choose an action to happen once the survey is submitted.
Sticky Contact: Enable sticky contact if you want Intellia to remember the user's details for future form submissions.
7. Save the Survey :
Once you're done adding and customizing questions, click the Save Survey button at the top right.
8. Embed or Use the Survey:
After saving, you can integrate the survey directly on a Intellia landing page or embed it on another website.
To embed the survey, click Integrate Survey, then copy the provided embed code.
Paste the embed code on your website's HTML section or use it in any custom HTML block if you're using platforms like WordPress, Shopify, etc.
